Executives, managers and supervisors use this application to better understand their organization’s performance on a variety of measures. They compare actual to target performance for key performance indicators (KPI) using case mix and severity adjusted targets, including statewide norms. KPIs include mortality rate, 30-day readmission rate, C-section rate, inpatient average length of stay (days), and emergency department average length of stay (hours). Results are presented in a trend analysis format, and for each KPI, users can drill down into service line and disease category, and can filter by time period.
Population Health Management
This application is designed to assist care managers and medical directors to apply healthcare resources to patients most at risk. This application includes a suite of dashboards helping the user understand the prevalence of disease across their patient population and identify populations and individuals most at risk of future disease, overall high costs, and care events such as ED visits and hospital admissions. Results are displayed in geo-maps and tables to allow for drill down into disease category, payer category, patient origin (zip code), and patient demographics. The application assigns each patient a risk score for each disease or care event, and users manage individual patients using lists that can be filtered by risk, disease, payer, and demographics.
Inpatient Readmission Risk / Emergency Visit Return Risk Management
This application is designed specifically to help manage patients at risk for 30-day hospital readmissions and repeat emergency department visits. Active and recently discharged patients are assigned a risk score showing their probability of a return visit within 30 days. Scores are updated in real time following admission and as new data is included during the patient’s stay. Along with these risk scores, users can access summaries that detail a patient’s 12-month clinical and encounter history, including risk score trending. Users can generate patient lists by disease and risk score as well as view retrospective 30-day readmissions by population, disease and facility.
This application shows the level of resource utilization variation across the organization and within certain patient populations. It helps executives and physician leaders identify the diseases, treatments and/or facilities with the greatest opportunity for variation reduction and cost improvement. Resource variation can be viewed based on charges, costs, procedures, tests, or length of stay, and provides targets for variation reduction and performance improvement. The application includes statistical measures of internal variation and variation against evidence based practice.

Executives and planning leaders use this application to support strategic and operational decision making. Users of this application can track and trend an organization’s volumes and market share by service area, disease, payer, and patient demographics. The application provides real time, geo-mapped displays for interactive visualizations of encounter volumes and market share as well as filtering by patient demographics, date range, disease, payer, and geography.
