Easy filtering can be done on age, multi-chronic conditions and risk of an inpatient admission or mortality to create a quick outreach list, rank ordered by probability of the event. Alternatively, clients can use a list of specific co-morbidities as defined by the CDC, researchers and front-line health care professionals, or have a custom cohort filter built.
Additionally, our acute risk clients can use risk for mortality during the encounter for triage or discharge guidance, enabling better allocation of resources.
